What Is Note IQ?
Note IQ is an AI-powered clinical documentation review tool built directly into Notenetic.
Once enabled, Note IQ automatically reviews supported documentation and provides quality insights, scoring, and recommendations to help staff improve documentation before approval, billing, or audit review.
What You’ll Need
- Feature enabled by Notenetic (beta access required)
- Access to Document Settings

Signing Up for Note IQ
Where to Go
- Navigate to More
- Select Subscriptions
- Locate Note IQ
- Select Sign Up
Subscription Confirmation
When selecting Sign Up, the following confirmation window will display:
Confirm Subscription Sign-Up
You are about to activate the following subscription:
- Plan: [Plan Name]
- Monthly Cost: $[Amount]/month
- Additional Fees: [If Applicable]
- Billing Start Date: [Date]
Your subscription will automatically renew each billing cycle unless canceled by your clinic administrator.

What Happens After Sign-Up?
Once the subscription is activated:
- The Note IQ subscription becomes active for the clinic.
- Enable Note IQ is automatically turned on within More > Settings > Document Settings.
- Super Admins can manage Note IQ visibility and template settings.
- Clinics can begin enabling Note IQ on supported document templates.
Canceling Note IQ
Clinic administrators can manage their Note IQ subscription from:
More > Subscriptions > Note IQ
Subscription changes may take effect according to your billing cycle and subscription terms.
Step 2: Enable on Document Templates
- Navigate to More
- Select Document Templates
- Locate the template you want to use
- Select the Settings (gear icon)
- Toggle on Enable AI Clinical Review
Note IQ will only review documents created from templates where this setting is enabled.
Customizing the Note IQ Review Prompt
Clinics can provide additional instructions to Note IQ to account for program-specific, payer-specific, or clinic-specific documentation requirements.
Where to Go
- Navigate to More
- Select Settings
- Open Document Settings
- Locate Note IQ Settings
- Enter your custom instructions in Add to Note IQ Prompt
Character Limit
- Maximum of 500 characters
- Character count will display while typing
How It Works
Information entered into the custom prompt is included as additional guidance during the Note IQ review process.
This allows clinics to provide context that may not be reflected in standard documentation requirements.
Examples
Clinics may use this area to provide instructions such as:
- Program-specific documentation expectations
- Required intervention documentation
- Payer-specific requirements
- Internal quality standards
- Clinic-specific terminology or workflows
Important Notes
- Custom instructions supplement the standard Note IQ review process
- Instructions should be clear and concise
- The 500-character limit includes spaces and punctuation
- Changes to the custom prompt will apply to future Note IQ reviews
How Note IQ Works
Once enabled, Note IQ automatically reviews documentation after submission.
Each document receives:
Overall Score Based On
- Completeness
- Coherence
- Medical Necessity
- Professional Standards
Score Ranges
- Good (≥ 75) – Meets expected documentation standards
- Fair (50–74) – May require minor improvements
- Needs Improvement (< 50) – Significant issues may be present
Feedback Categories
- Critical – Issues that should be addressed before approval or billing
- Warning – Areas that may impact quality or compliance
- Suggestion (Info) – Opportunities to improve clarity or detail
AI Output Includes
- An overall score
- A detailed assessment summary
- Findings and recommendations
- A breakdown of issues by severity
Where to View Note IQ Results
Note IQ results are available in multiple areas:
1. Within the Document
- A Note IQ panel appears on the right side
- Displays:
- Overall score
- Category scores
- Assessment
- Findings and recommendations
2. Documents List View
- Reviewed documents display a Note IQ score
- Hovering over the score shows:
- Visual score indicator
- Summary of findings
This allows staff to quickly identify documents that may need attention.
3. Notifications Panel
- Users receive real-time notifications when reviews are completed
- Notifications include:
- Document reference
- Review completion status
- Category scores
For Clinicians (Submitting Documentation)
- After submitting documentation, the note is automatically reviewed
- Once complete, clinicians will receive:
- A popup notification (upper right)
- A message in their Message Center
- Results can be viewed:
- In the document
- From the Message Center
- From the Documents list
For Document Reviewers
- Reviewers can send batches of documents for AI review
- As reviews complete, they will receive:
- Live notifications
- Messages in the Message Center
- A new “Note IQ” tab is available in Documents
- Displays all reviewed documents
Using Note IQ in Your Workflow
- Prioritize low-scoring documents first
- Address critical findings before approval or billing
- Use warnings and suggestions to improve overall documentation quality
- Use the Documents list view to quickly identify documents needing attention
Do I Need to Fix Note IQ Findings?
Note IQ is a support tool and does not automatically block submission.
- Documents may still be submitted unless otherwise configured
- However, it is strongly recommended to:
- Address critical issues
- Review findings before billing or approval
Clinics may establish internal policies for how Note IQ results are used.
Why Wasn’t My Document Reviewed?
If a document was not reviewed, check the following:
1. Feature Not Enabled
- Note IQ must be enabled in Document Settings
2. Template Not Enabled
- The document template must have Enable AI Clinical Review turned on
3. Document Not Submitted
- Only submitted documents are reviewed
- Drafts are not included
4. Processing In Progress
- Reviews may take a short time to complete
- Notifications will appear once finished
5. Beta Limitations
- Some document types or configurations may not be included in the current beta
Still Not Seeing a Review?
- Refresh the page
- Check your Message Center
- Confirm settings with your Super Admin
- Contact support if needed
Important Notes
- Note IQ is a support tool and does not replace clinical judgment
- Only documents created from enabled templates will be reviewed
- Review timing may vary depending on system processing
