Please note: The ability to add a diagnosis to a client is based on your role/access level.
If you are unable to follow the steps below, please reach out to your Super Admin to review your permissions.
There are two ways to add a diagnosis to a client’s profile.
Option 1: Add Diagnosis from the Client Profile

Go to Clients and use the search or filters to locate the client in question.

Click to view the client’s profile.

Select the Diagnosis tab on the left-hand menu.

Click Add Diagnosis.

In the pop-up, input the following details:

Diagnosis Date
-
Diagnosis Code or Description

(Optional) Checkboxes for:

Ruled Out Dx? — if the diagnosis was considered but ruled out

Historical Dx? — if the diagnosis is no longer current but should be retained for record-keeping

Click Add to save the diagnosis.
Setting the Primary Diagnosis
After you’ve added a diagnosis:

Use the hamburger icon (three lines) on the left of the appropriate diagnosis

Drag and drop it to the top of the list to make it the primary diagnosis

New Feature: Diagnosis Education Material
After adding a diagnosis, when viewing the Diagnosis tab in the client’s profile, you’ll now see diagnosis education available for each diagnosis listed.
Staff can choose to download, email, or print these educational resources to share with the client.
Option 2: Add Diagnosis via Document Template (Requires Super Admin Setup)
This option allows a diagnosis to be saved to the client profile when submitting a document.

Go to More > Template Settings

Locate the document template you want to enable for diagnosis saving

Scroll to the far right and click the gear icon

In the pop-up settings modal, toggle on:

✅ Show Dx (Save to Client Profile)

Click Save
Once this is enabled, staff will be able to select a diagnosis when completing the document, and it will automatically save to the client profile upon submission.
