How To: Adding Social Determinants

Social Determinants Added:

To access Social Determinants of Health (SDoH) for a client please follow the below instructions.

1. Go to a clients profile
2. Select Background option on the left.
3. Then select, "Add Social Determinants"
4. Once SDoH is completed there is an "Add" button at the bottom
5. The results to the Social Determinants will display on the clients dashboard.
